Testosterone is often described simply as the male sex hormone, but that description barely scratches the surface of its importance. It is a steroid hormone produced primarily in the testes that influences everything from muscle mass and bone density to mood, cognition, libido, and metabolic function. Understanding how testosterone works is the first step toward managing your health proactively.

Where Testosterone Comes From

The production of testosterone is regulated by the hypothalamic-pituitary-gonadal (HPG) axis. The hypothalamus releases gonadotropin-releasing hormone (GnRH), which signals the pituitary gland to release luteinizing hormone (LH). LH then travels to the testes, specifically to Leydig cells, which produce testosterone in response. A feedback loop keeps levels in balance: when testosterone is sufficiently high, it signals the hypothalamus and pituitary to reduce production. This is why exogenous testosterone can suppress natural production.

A small amount of testosterone is also produced by the adrenal glands in both men and women. In women, testosterone plays important roles in libido, bone health, and energy, though at roughly 10-15% of male levels.

What Testosterone Does in the Body

Testosterone has two broad categories of effects: androgenic (masculinizing) and anabolic (tissue-building). Androgenic effects include the development of male characteristics — facial hair, deepening of the voice during puberty, genital development, and sex drive. Anabolic effects include stimulating muscle protein synthesis, increasing bone mineral density, and promoting red blood cell production.

Beyond these well-known roles, testosterone also affects insulin sensitivity, fat distribution, cognitive function, mood regulation, and cardiovascular health. Low levels have been linked to increased visceral fat, insulin resistance, depression, and higher cardiovascular risk.

Key fact: Testosterone levels are highest in the morning — typically 20-25% higher than in the evening. This is why blood tests for testosterone are standardized to be drawn between 7 a.m. and 10 a.m. for accurate results.

Normal Testosterone Ranges

Reference ranges vary by laboratory, but total testosterone in adult men is generally considered normal between 300 and 1,000 ng/dL. Free testosterone (the biologically active fraction not bound to proteins) typically ranges from 50-210 pg/mL. Levels decline with age at roughly 1% per year after the mid-30s — a process sometimes called andropause. By age 70, average levels are about half of what they were at age 25.

The Difference Between Total and Free Testosterone

Most testosterone in the blood (about 97-99%) is bound to proteins, primarily sex hormone-binding globulin (SHBG) and albumin. Only the remaining free testosterone is immediately bioavailable to tissues. This is why a man can have "normal" total testosterone but still have symptoms of low T if SHBG levels are elevated, leaving insufficient free testosterone. A complete workup should include both total testosterone and SHBG measurements, from which free testosterone can be calculated.
